Referral Form - Completed by referrer with consent of parent/carer & young person.
Referral from other professional completed & consent of parent/carer & young person. Reason for referral meeting the above criteria. Case allocated to team member and contact made with young person, completion of ONSET assessment - Individual Support Plan will be drawn up and an agreement to engage in this process.

1:1 support to children and young people who have been a victim of crime, bullying, involved in low level anti social behaviour, 1st time entrants to the criminal justice system.
Indiviudal support to children and young people. Tailored to meet the identified and emerging needs, generally weekly contact for 1 hour for a period of between 6 weeks to 6 months.
Service is provided at the office base, within the young persons school, or another suitable venue, determined by a number of factors. Service is delivered between 9am - 6 pm.
